Required Documents:

1. Official Dental School transcript - must be submitted directly by the school to RIDOH
2. Official copy of the National Board Scores - must be submitted directly by the American Dental Association (ADA) (312) 440-25003.
3. Official copy of the Regional or State Board examination results - Must be sent directly from the licensing board to the RIDOH
4. Copy of current Drivers License/State ID
5. A current certification from an approved course in Basic Cardiac Life Support, (BCLS), Advanced Cardiac Life Support (ACLS), and/or Pediatric Advanced Life Support (PALS)
6. Self-Query from the National Practitioner Data Bank
